Deprescribing in Real Time: Hospitalized Septuagenarian With Polypharmacy

Polypharmacy is a common and potentially preventable contributor to recurring emergency room visits, hospitalization, morbidity, mortality. Its consequences are magnified in older adults due the age-related decrease functional physiologic reserves, increased blood-brain barrier permeability, altered drug metabolism, among others. In this article, we describe case of polypharmacy septuagenarian highlight deprescribing approach implemented by inpatient care team offer patient-centered insights clinicians (primary providers hospitalists) when making decisions. The overarching aim article build on existing literature regarding polypharmacy, prescribing cascades, context what matters most aligns with patient health priorities. This highlights importance good geriatric medication reconciliation stewardship avoid harm.
